Abstract     The most frequent indication for surgical treatment of chronic pancreatitis is intractable pain. In patients with a dilated main pancreatic duct and absence of inflammatory tumour in the head of the gland, the most suitable method to control the pain is longitudinal pancreaticojejunostomy. When the head of the pancreas is enlarged showing inflamatory changes, a resection of the head of the pancreas, such as partial pancreatoduodenectomy and pyloruspreserving partial pancreatoduodenectomy is indicated. Resection of the tail of the pancreas is indicated only in cases where serious complications are present in this part of the gland like pseudoaneurysms. A significant improvement in the patients quality of life is achieved with the combinations of extended surgical drainage and limited resection of the gland. When pancreatic carcinoma is suspected in a patient with chronic pancreatitis, the diagnosis is confirmed or excluded by intraoperative histologic proof.
