Abstract     Background. Radical operation for pancreatic cancer is the only procedure offering the patients the possibiliiy of cure. In nonresectable tumours bilioenteric derivation ensures better quality for the rest of the life. The author's own 25-year series of operated patients is presented. Patients and methods. In the period from Jan. 1976 to Dec. 2001 author operated on 147 patients with pancreatic cancer: 69 patients underwent presumably radical or only paliative procedure (61 patients had pancreatic head cancer, 6 patients adenocarcinoma of body or tail of pancreas and 2 had cystadenocarcinoma). ln 60 patients pancreatoduodenectomy, in 8 cases subtotal pancreatectomy and in one patient total pancreatectomy were performed. Results. During postoperative course 7 patients with pancreatoduodenectomy had to be reoperated for surgical complications. Three patients died in the 30 days after operation - postoperative mortality rate 4, 3%. Five patients with pancreatoduodenectomy lived longer than 5 years (107, 100, 87, 80 and 69 months) and one patient aher subtotal pancreatectomy for cystadenocarcinoma. At the end of 2001 8 other resected patients (7 after pancreatoduodenectomy and 1 with left subtotal pancreatectomy for ductal adenocarcinoma) were alive. At that time they had not yet fulfilled 5 years aher surgery. Five year survival for resected patients in this series is 22% (Kaplan-Meier). There are no data on postoperative complications, mortality and survival for the nonresected or only explored patients. Conclusion. Radical resection of pancreatic cancer offers the patient a small possibility of cure and bilioenteric derivation in nonresectable tumours gives the best possible quality for the rest of life.
