Abstract     A family physician is often faced with the patients who suffer from a shoulder pain. A pain in the shoulders is an irritating problem for the patients and it offers a big diagnostic challenge to the physicians due to a complex structure in this joint. For understanding of the problem knowledge about joint anatomy is paramount importance. Several diseases provoke the shoulder pain. Often ill defined problems from origins out of the joint structures cause the pain in the shoulder. Patophysiology is often unclear in spite of the diagnostic nomenclature close to skeleton (tendinitis, capsulitis, frozen shoulder). Besides these, the pain may irradiate from sources outside joint structures (a head, neck, diaphragm, abdominal and chest structures). The diagnostics and treatment of the painful shoulder is in the hands of the family physician. If the pain and the constrains of shoulder movements persist for a few months without definitive diagnosis, the patient should be referred to an orthopedic surgeon for a definitive care. Younger patients in the case of suspected rupture of a tendon should get an earlier referral to the specialist. In the presence of the rheumatic disease shoulder pain is treated as a part of the syndrome.
