Abstract     The clinical application of serum bactericidal activity test was evaluated. The serum bactericidal activity test identifies the greatest dilution of a patient's serum which can kill the causative pathogen. The study was carried out in order to correlate the successful treatment and the bactericidal activity of the serum. The statistical correlation was good. The specificity of the serum bactericidal activity test was found to exceed its sensitivity. The test was shown to have the greatest accuracy if titer values were 1:2 and greater. At attempt was made to compare changes of the body temperature and the white cell count in peripheral blood with the therapeutic outcome and the bactericidal activity of the serum. In five cases out of 42 (12 percent) changes of body temperature and the white cell count observed during the initial 48 hours after the start of therapy did not agree with the ultimate treatment outcome. In two cases of them bactericidal activity was the only parameter that correlated with the final outcome. The bactericidal activity and the concentrations of antibiotics in the serum were determined in 21 cases. Analysis revealed lack of agreement between the two parameters.
