Abstract     The authors summarise development of malleolar fractures treatment, their classification, physical and radiograph diagnostic. Indications for operative treatment present fractures with incongruence of ankle mortise or joint cartilaginous surfaces with shortening of fibula and unstable ankle joint. Treatment of fractures type A by Danis- Weber and same transysindesmotic fractures type B without dislocation is conservative. Fractures type C present indication for ORIF. According to the shape of fracture osteosynthesis with compression screws, Kirschner wires and small fragmental plate is recommended. Minimal invasive osteosynthesis is indicated in case of serious comminuted fractures, by patients with bad peripheral circulation, severe osteoporosis, diabetic patients and by skin and soft tissue problems.
