Abstract     BIood bags for collection, processing, storage and administration of blood and blood components are manufactured from one or more polymers, if necessary with additives. The usually used polymer for blood bags is polyvinylehloride, mostly plasticised with phtalic acid esters especially with di(2-ethylhexyl) phthalate (DEHP). DEHP is not chemically bound to the plastic and it can migrate into stored blood and blood components. It is well known that DEHP causes reproductive and hepatocarcinogenic effect in rats and mice. The potential cancerogenicity for human, especially for patients receiving massive transfusions and for dialysis patients was not confirmed till now. On the other hand, scientists found out that DEHP stabilizes the membrane of red blood cell. It incorporates into the area that was vacated by the lipid and it bridges the asymetric bilayers. In this way the spontaneous hemolysis of red blood cells is reduced and the survival of red blood cells during storage is improved. For the storage of platelets blood bags of polyolefins are more suitable, because they are more gases permeable than polyvinylchloride blood bags. Quality control of blood bags is performed according to pharmacopoeia.
Summary     Za zbiranje, predelavo, shranjevanje in aplikacijo krvi in krvnih sestavin se uporabljajo izključno sistemi vrečk za kri, ki so izdelani iz enega ali več polimerov in, če je potrebno, tudi z dodanimi aditivi. Najpogosteje uporabljen polimer je polivinilklorid, ki je plastificiran z estri ftalne kisline, še posebno z dietilheksilftalatom. Dietilheksilftalat ni kovalentno vezan v polimeru in se lahko izloča iz plastike v kri in krvno sestavino v času njenega shranjevanja. Znano je, da dietilheksilftalat povzroča hepatokancerogene učinke pri podganah in miših. Potencialna toksičnost pri človeku, predvsem pri bolnikih, ki dobivajo masivne transfuzije, in pri dializnih bolnikih še ni dokazana. S številnimi raziskavami pa so zanstveniki po drugi strani odkrili, da dietilheksilftalat stabilizira membrano eritrocita. Vgradi se v membrano na področje, kjer manjka lipid, in poveže asimetrični dvosloj membrane. Na ta način se zmanjša spontana hemoliza eritrocitov in izboljša preživetje eritrocitov v času shranjevanja v vrečki. Za shranjevanje koncentrata trombocitov so primernejše vrečke iz poliolefinov, ker so bolj propustne za pline kot vrečke iz polivinilklorida. Kontrolo kakovosti vrečk za kri izvajamo po farmakopeji.
