Abstract     Food allergy can manifest itself as a variety of gastrointestinal symptoms, signs and syndromes. The pathogenesis usually involves immune reactions of cell-mediated immunity, with only a minority of reactions being IgE mediated. The diagnostic approach to a patient with suspected food allergy and with signs of gastrointestinal involvement therefore requires a good knowledge of clinical syndromes of gastrointestinal allergy. The final diagnosis must be based on a thorough history, physical examination and elimination/provocation tests. Histologic examination of biopsy specimens taken from several regions of the gastrointestinal tract can be a valuable diagnostic tool. Treatment is based on an elimination diet. Highly hydrolysed formulas are recommended in infants and young children. Patients that are allergic to these hydrolysed formulas require amino-acid based formulas. It is very important that a diagnosis of food allergy is based on objective parameters. A falsely established diagnosis can lead to serious malabsorbtion, eating disorders and psychosocial problems. An overlooked diagnosis, however, presents an unnececary burden and can lead to serious disabilities in a child.
