Abstract     Background. To study the long term neurodevelopmental outcome of children with very short gestational age (25 weeks or less) who were treated with steroids postnatally. Methods. Seventy-six babies with gestational age 22-25 weeks who were born alive in Ljubljana Maternity Hospital in the four year period of 1997-2000 were included in the study. Thirty-two (42%) survived to discharge. Followup data were obtained on 29 children at a mean corrected age of 18 months. With regard to the neurological outcome (normal, moderate ar severe neurological disability) antenatal, perinatal and neonatal factors of the followed infants were analyzed. Results. Five of 29 infants had abnormal neurosensory development (17%) and 3 more were still at risk for developmental delay. Infants with abnormal or at risk development had similar prenatal and perinatal risk factors (bleeding, tocolysis, steroids; breech presentation, cesarean section), were of the same birth weight and gestational age. They had similar Apgar scores 1 and 5 minutes after birth and the need for ventilatory support in neonatal period. However, we found significant difference between the groups with regard to abnormal cranial ultrasound findings (grade 3-4 intraventricular haemorrhage: p<0.01; periventricular leukomalacia; p<0.05) and to postnatal steroid treatment (p<0.05). Conclusions. Postnatal steroid treatment has adverse effect on neurological outcome in infants born between 23 and 25 week of gestation.
