Abstract     Systemic lupus erythematosus (SLE), rheumatoid arthritis (RA) and systemic sclerosis (SSc) are autoimmune diseases that predominantly affect women of reproductive age. Pregnancy and its outcome are a major concern to most SLE, RA and SSc patients, Queries regarding the risk of flare-ups of the disease during pregnancy, chance of foetal loss and the safety of various drugs are often raised. With the improvement in the understanding of the pathogenesis of these autoimmune diseases and the judicious use of immunosuppressive drugs, better disease control can now be achieved and SLE, RA and SSc patients should not be deprived of the opportunity of bearing children. Prepregnancy counselling and close collaboration with other specialists is essential in optimising the maternal and foetal outcome. In this review, important issues regarding optimal timing of conception, risk of disease flare-ups during pregnancy, pregnancy course, foetal outcome, safety of various drugs used for disease control during pregnancy and lactation are discussed.
