Abstract     Pathological changes in transplanted kidney may present donor kidney pathology before transplantation (donor kidney disease, damage due to shock or warm and cold ishaemia, perfusion damage, infection of the kidney). The changes may follow various surgical complications (fistulas, urinary obstruction, uroinfection, lymphocele, transplant rupture, arterial or venous thrombosis etc.). The renal pathology may be direct consequence of different forms of transplant rejection (acute or chronic, due to humoral or/and cellular immune response), or they reflect indirect connection with renal transplantation (transmission of donor kidney disease, recurrence of original recipient's kidney disease or the novo glomerular disease in renal transplant). Immunosuppressive therapy is also involved in the development of some renal changes (cyclosporine or tacrolimus nephrotoxicity, infections - cytomegaly, polioma virus, mycoses, lymphoproliferative disorders, de novo diabetic nephropathy in renal transplant). The changes may develop due to different successively or simultaneously acting above-mentioned etiologic factors. The last updated Banff classification of the changes in renal allograft is presented.
