Abstract		Background. Since 1996 there have been 446 patients (107adults) operated for strabismus. This retrospeetive study (retrospection) indicated that out of those 107 adults 22 of them suffer from convergent concomitant strabismus, 29 from convergent paralytic strabismus, 42 from divergent concomitant strabismus and 14 from divergent paralytic strabismus. Three cases of strabismus surgery on adult patients are presented. Strabismus is a pathological deviation of one eye turning outwards, inwards, upwards or downwards that has severe consequences for sight and aesthetic appearance of a patient. Methods. Therapy consisted of ambulatory patient care, surgery and postoperative orthoptic pleoptic training. In the first case of convergent paralytical strabismus with vertical component, the retroposition of internal rectus muscle, retroposition of inferior rectus muscle and resection of upper rectus muscle were performed. In the second case of divergent strabismus, resection of both internal rectus muscleand retroposition of external rectus muscle on left eye were performed by means of one surgery. In the third case of traumatic divergent paralytic strabismus, the late reconstruction and then resection of internal rectus muscle and retroposition of the external rectus muscle were performed. Results. Through operative treatment the objective squint angle was diminished and the aesthetic appearance was improved in all three cases. Conclusions. By means of strabismus surgery the aesthetic appearance of adult patients can be improved and by paralytic strabismus weak sight (amblyopia), contraction of the eye muscles, ocular torticolis and psychic trauma can be prevented. ln order to achieve a permanent result with surgical operation, the continuated postoperative orthoptic pleoptic training is necessary.
Summary		Izhodišča. Od leta 1996 do 2003 smo strabizem operirall pri 446 bolnikih, od tega 107 odraslih. Z retrogradna študijo smo ugotovili, da je bilo od teh 107 operacij 22 konvergentnih konkomitantnih (spremljajočih) strabizmov, 29 konvergentnih paralitičnih (nespremljajočih) strabizmov, 42 divergentnih konkomitantnih in 14 divergentnih paralitičnih strabizmov. V našem prispevku bomo predstavili tri različne primere operacij strabizma pri odraslih bolnikih. Strabizem je patološki odklon enega očesa navzven, navznoter, navzgor ali navzdol in ima težke posledice za vid in estetski videz bolnika. Metode. Zdravljenje je bilo ambulantno, operativno in postoperativno z ortoptično pleoptičnimi vajami. V prvem primeru predstavljenega konvergentnega paralitičnega strabizma z vertikalno sestavino smo izvedli retropozicijo notranje preme mišice, retropozicijo spodnje preme mišice in resekcijo zgornje preme mišice. V drugem primeru opisanega divergentnega konkomitantnega strabizma smo z eno operacijo izvedli resekcijo obeh notranjih premih mišic in na levem očesu retropozicijo zunanje preme mišice. V tretjem primeru opisanega travmatskega divergentnega paralitičnega strabizma smo izvedli kasno rekonstrukcijo in nato resekcijo notranje preme mišice ter retropozicijo zunanje preme mišice. Rezultati. V vseh treh primerih smo z operacijo strabizma zmanjšali objektivni škilni kot in izboljšali estetski videz Zaključki, Z operacijo strabizma pri odraslih bolnikih lahko izboljšamo estetski videz, pri paralitičnih strabizmih preprečimo slabovidnost, kontrakturo notranje preme mišice, prisilno držo glave in psihične motnje. Za trajno uspešnost kirurškega posega pa so potrebne kontinuirane postoperativne ortoptično pleoptične vaje.
