Abstract     18 years old drug addict was admitted with hypovolemic shock after a drug overdose. He deteriorated to cardiac arrest in the Emergency Department and was successfully resuscitated. Subsequently he developed rhabdomyolysis, liver impairment, and kidney and respiratory failure. Liver impairment was caused by ischaemic hepatitis secondary to hypovolemia and cardiac arrest. Liver function tests indicated direct hepatocellular damage with high ALT (3278 IU/I), AST (5514 IU/p, hypoglycemia, and only minor elevation of bilirubin (6 micromol/l) and alkaline phosphatase (174 IU/p. As a consequence, the patient became hypoalbuminemic (13 g/I) and coagulopathic (INR 16.9, APTT 300 s, platelets 41 x109/I). With aggressive supportive treatment and despite multiple organ failure he made a complete recovery and was discharged from the hospital after 14 days.
