Abstract     In current guidelines for the management of adults with community-acquired pneumonia (CAP) the assessement of pneumonia severity is of most importance. It is not only crucial for decision of hospital or even ICU admission, but also for selection of initial anfibiotic therapy. The severity cryteria of American and British Thoracic society (ATS,BTS) are used in our practice. CAP-course with sepsis indicates hospital treatment, often even in intensive care unit. The decision enables clinical and laboratory cryteria. The artikel present the role of host defence factors in septicaemia. Risk factors for complicated course of CAP are largely exposed also in sense of decision for initialy empiric therapy. Microbiologic haemoculture results are especialy stressed as the key guide in treatment of severest cases. Hospitalised cases of CAP with positive haemocultures in Clinic of Chest Diseases and Allergy in period 2000-2002 are presented as well. General treatment measures, forming also the newly used »Clinical Pathway« for CAP, are also described.
