Abstract     Introduction. End-stage renal failure and chronic hemodialysis induce a state of immunodeficiency and increased infection risk seen in these patients. Case presentation. We report a case of a 58-year old patient with end-stage renal failure, treated with hemodialysis for the last year, who was admitted due to pain in the lower abdomen: During the diagnostic procedure emphysematous gastritis and acute cholecystitis were confirmed. Acute , abdomen developed and the patient underwent emergency laparotomy. During the procedure necrotic gallbladder was removed. The patient died the following day due to multiorgan failure and septic shock. Conclusions. Emphysematous gastritis is according to the literature an almost uniformly fatal disease.
