Abstract     Background. According to GINA guidelines asthmatic patients should be (nearly) without asthmatic symptoms, have minimal requirements for reliever, without limitations to daily activities and with appropriate quality of life. The upper mentioned goals of asthma therapy were not met in everyday life and a failure of the GINA expectations was confirmed by epidemiological studies. This study presents a management of adults and children with asthma in Slovenia. Methods. Patients with asthma were chosen at random from all regions of Slovenia. 172 adults and 168 children with asthma filled in a questionnaire. Results. Our patients were prescribed inhaled glucocorticoid (85% of adults and 96% of children with asthma) in much greater segment than asthmatic patients in other countries. Still they had too many daily asthmatic symptoms and experienced too frequent episodes of nocturnal asthma. Even trivial activities were significantly disturbed by asthma. Their quality of life was low. Our patients admitted low compliance with therapy with inhaled glucocorticoids. Conclusions. In spite of appropriate prescriptions of inhaled glucocorticoids to asthmatic patients included in our study the goals of asthma therapy were not met. The most obvious reason for that failure was low compliance to therapy with inhaled glucocorticoids. It is belived that intensive education programs for asthmatic patients and the medical personnel involved in the management of asthmatic patients should be proposed.
Summary     Izhodišča. Bolniki zastmo naj bi bili po svetovnih smernicah za obravnavo astme skoraj brez simptomov, normalno telesno zmogljivi in s primerno kakovostjo življenja. V vsakdanji praksi se je izkazalo, da ni tako. Neugodne izide zdravljenja astme so potrdili v obsežnih epidemioloških raziskavah. V raziskavi smo ugotavljali, kako so vodeni bolniki z astmo v Sloveniji. Metode. Po metodi naključnega izbora smo izbrali iz vseh regij Slovenije 172 odraslih in 165 otrok z astmo. Bolniki so izpolnili obsežen anketni list. Rezultati. Bolniki so navajali bistveno preveč dnevnih in nočnih simptomov astme. Astma jih je ovirala pri mnogih, tudi dokaj trivialnih opravilih. Izmerili smo jim opazno slabšo zdravstveno kakovostživljenja in pomanjk jivo znanje o astmi. Bolniki so imeli inhalacijski glukokortikoid predpisan v opazno večjem odstotku, kakor so ugotovili v tujih presečnih študijah (85% odraslih in 96% otrok z astmo). Vendar se povečani predpis inhalacijskega glukokortikoida ni zrcalil v izboljšanih izidih zdravljenja, ki so enako neugodni, kot jih navajajo drugi raziskovalci. Zaključki. Bolniki z astmo, ki so bili vključeni v raziskavo, so imeli večinoma predpisan inhalacijski glukokortikoid. Vendar bogato predpisan inhalacijski glukokortikoid ni pripomogel k boljši obravnavi preučevanih bolnikov z astmo. Med vzroke za neuspeh zdravljenja moramo šteti slabo zavzetost za zdravljenje, slabo znanje o astmi in napačno presojo o urejenosti astme. Menimo, da je skupni imenovalec naštetim vzrokom preslaba poučenost preučevanih bolnikov o astmi. Menimo, da je treba širiti kakovostni pouk o astmi.
