Abstract     120 primigravid women due to undergo early pregnancy termination (6.5 to 100) were randomly selected for prostaglandine application 6 hours prior to vacuum aspiration. Carboprost (0.25 mg) was applicated intramusculary to 60 women, and 0.5 mg of dinoprostonein the form of intracervical gel was applicated to the other 60 women. Before application all women had the cervix closed, i. e. dilated less tha 3 mm. After 6 hours a marked cervical softening and dilatation was noticed in all 120 cases (100 per cent ). In the dinoprostone group of women, dilataton was statistically significantly greater than in a charboprost group. In 44 (73.3 per cent ) women in the carboprost group gastrointestinal side effects occurred, whereas in the dinoprostone group gastrointestinal side effects were observed in 28 (46.7 per cent ) cases. After prostaglandine application there were no significant changes in blood pressure and body temperature. During the procedure there were no complications. We can conclude that application of prostaglandins is a very effective method for preoperative dilatation and prevention of cervical and uterine injury wich can occur in mechanical dilatation. Side effects occur quite often, especially when systemic application is used, but can be successfully treated with apropriate medication. Preoperative application of prostaglandine should be used always in primigravid women when pregnancy termination is desired.
