Abstract		In 25 patients suffering from urogenital atrophy treatment with Vagifem, containing 25 microg of estradiol in one vaginalette was performed. The stages of urgenital atrophy were estimated according to the results of cytohormonal smears before treatment and two and 12 weeks during the treatment. In the same time periods gynecologic and breast examinations were performed and blood pressure measurement and body weight were checked. The patients were aged from 43 to 70 years, in the postmenopause from 1 to 20 years. After two weeks of daily usage of Vagifem intense proliferation of vaginal epithelium was registered with an increase in superficial cells and appearance of eosinophilia and karyopiknosys. In twelve weeks of therapy local symptoms were absend, although maturation index, eosinophilia and kariopicnosys decreased because of intermittent treatment performed twice weekly. Blood pressure and body weight remained in the majority of patients unchanged. Decrease of body weight was registered in 5.2 per cent of patients while increase was found in 21.1 per cent . Controlled release of estradiol from hidrophilic-cellulose matrix is an advantage concerning treatment used untill now. Because of evenly distribution on vaginal mucosa there is no local accumolation and consequently no possibilty of episodic estrogen rise in serum.
